How are NPCs spawned?

Hello all,

I'm chasing down some bugs and I think one might be related to when a NPC is spawned. I know some NPCs are spawned by the AREAXXXX.bcs, but some are not (Branwen, Minsc). When / where / how are these spawns defined?
